Glyburide, also known as optimal hypoglycemic and dayantherapy, belongs to the second generation of oral sulfonylurea hypoglycemic drugs, and its action mechanism is the same as toluensulfonylurea. Its hypoglycemic effect is the strongest among sulfonylurea, and its action intensity is 200-250 times that of toluensulfonylurea. It can selectively act on pancreatic β cells and promote the secretion of insulin, enhance the hypoglycemic effect of exogenous insulin and enhance the post-receptor effect of insulin. Oral absorption is fast, protein binding rate is high, the effect begins at 30 minutes, the strongest effect at 1.5 hours, the duration is 16-24 hours. The distribution volume is 0.1L/kg, the binding rate of plasma protein is 90% ~ 95%, and the half-life is 4 ~ 8 hours. It is mainly metabolized by liver and contains 6 metabolites, 2 of which are known to be hydroxyl compounds, which have no hypoglycemic effect and are mainly discharged from urine and a small amount from stool. Clinically, it is mainly used for the treatment of mild and moderate non-insulin dependent diabetes mellitus. An international study has found that the commonly used diabetes drug Glyburide helps the body's immune system fight certain bacterial infections, cutting deaths by about half when used to treat melioidosis. Melioidosis is a type of disease prevalent in tropical regions such as Southeast Asia and northern Australia. It is caused by the bacterium Melioidosis. Symptoms include septicemia and pneumonia, and the death rate is high. People with diabetes tend to be more susceptible to melioidosis, but have a lower mortality rate than other patients.
